- Complete the referral form online on our Referral Client Portal – login here. To find out more about our Client Portal, please email info@emergingtherapies.com.
- Submit a completed offline Referral Form via email to medicalservices@emergingtherapies.com or fax to 612-445-5649.
- Or, simply call our medical services team at 877-445-4822 and we’ll be glad to help with your referral.
